Pa. hospital's high-risk care teams help reduce readmissions

Allegheny Valley Hospital near Pittsburgh cuts its readmission rate from 19.2% in 2012 to 11.8% this year using high-risk care teams that visit patients at home. The teams usually include nurses, social workers and paramedics and compile daily reviews of discharged patients to determine which ones might benefit from a visit.
