New models for care address social aspects of chronic disease risk

New care models such as patient-centered medical homes, medical neighborhoods and ambulatory intensive care units aim to uncover and help patients address underlying social factors in diabetes, heart disease, obesity, asthma and other chronic illnesses. The models compensate health care providers for connecting patients with community organizations and for addressing patients' access to safe housing and neighborhoods, places to exercise and healthy foods.
