Engineer detected seal failure in BP well ahead of Gulf blast

Jesse Gagliano, an engineer with Halliburton, said he told BP that a key cement seal inside its Macondo well might break down, his warning coming five days before the Gulf of Mexico rig blast and oil spill. Speaking before a federal panel probing the incident, Gagliano said that if not addressed, there was a "high probability" of a natural gas surge through the cement. BP disregarded his advice, he added.
