Big insurers say they will scale back Medicare Advantage plans

Several of the biggest health insurers in the U.S. have said they intend to withdraw from some Medicare Advantage markets as they prepare for changes under the Affordable Care Act. The law will impose an 85% medical loss ratio on such plans and is expected to reduce Medicare Advantage payments.
