Survey: Communication issues found in nursing home transfers

University of Wisconsin researchers who interviewed 27 nurses in skilled nursing facilities said poor communication is common during transfers of elderly patients from hospitals. Most nursing home nurses said they often lack information on patient's clinical status and on what happened during the hospitalization, according to the study in the Journal of the American Geriatrics Society.
