N.Y. hospitals test efficacy of transitional care units

Hospitals in New York state are opening transitional care units as part of a demonstration project to help patients successfully care for themselves at home once they leave the hospital. TCUs are seen as a way to provide better transition care at less cost than other rehab centers, as well as improve outcomes and reduce emergency department visits and rehospitalizations.
