Report cites design flaws in Florida parking garage collapse

A government report found that serious design problems contributed to the collapse of a Jacksonville, Fla., parking garage under construction in 2007. The six-level structure collapsed during the concrete placement for its top level. The Occupational Safety and Health Administration report cites failures by the project's subcontractor for vertical concrete, formwork and shoring work. Elements of the parking garage were "under-designed," according to the report.
