Report: Big Dig managers advised fewer bolts in tunnel

Project managers on Boston's Big Dig project convinced a designer in 1998 to reduce the number of bolts supporting each ceiling hanger in the Interstate 90 connector tunnel from four to two, according to a Boston Globe report. Sections of the tunnel remain closed after panels in the tunnel fell, causing one fatality.
