Medical errors harm millions of people worldwide every year

About 43 million cases of patients being harmed by adverse events are recorded due to substandard hospital care each year worldwide, adding up to 22.6 million years of life lost to death or disability, according to a review in the journal BMJ Quality and Safety. Harm from prescription drugs was the most common adverse event in high-income countries, while blood clots were the adverse event seen most often in low- and middle-income countries.
