Clinic's medical home model brings schools into care plan

The pediatrics department at Mankato Clinic in Minnesota is using a patient-centered medical home model to coordinate a child's care across providers and even into the classroom. A nurse practitioner works with the patient and family to set up a care plan so transcripts of health care visits are sent to all providers and educators are kept up-to-date on interventions or medications the child will need at school.
