Health care model for children with chronic illness includes schools

The pediatrics department at Mankato Clinic in Minnesota is using a model of care for young patients that is coordinated across health care providers and schools. In most cases, a nurse practitioner works with the patient and family to set up a care plan. Doctors can access transcripts of health care visits and medications in one place, and they are made aware of interventions being used with the child at school.
