Physician describes group practice's transition to PCMHs

Northeast Georgia Physicians Group is in the process of changing its physician practices into patient-centered medical homes, and chief physician executive Antonio Rios, M.D., said one challenge has been getting physicians to change their thinking from a single-visit care model to being responsible for a panel of patients. The group is using data analytics to help improve the number of patients who have their diabetes under control, and Rios said physicians want technology that provides immediate data, rather than relying on historical claims-based information.
