Hospitals focus on readmission rates to avoid Medicare penalties

In states across the U.S., hospitals are stressing the need for effective discharge plans and post-discharge care to reduce their readmission rates and avoid or reduce any penalties from their Medicare reimbursements, as prescribed by the Affordable Care Act. In Missouri, for example, 29 hospitals face increased penalties, 32 will receive reduced penalties and 20 will not face any penalty.
