Report: Hospitals lack response plans for adverse events

Many hospitals don't have a plan to deal with serious clinical adverse events, according to a report by the Institute for Healthcare Improvement and the Harvard School of Public Health. The report offers suggestions for creating response plans as well as checklists, questions to ask about preparedness, and language to use with the public and staff following an adverse event.
