Tesoro faces $2.38M penalty for Wash. state refinery explosion

Washington state's Department of Labor and Industries has levied a $2.38 million penalty on Tesoro for a deadly refinery blast in April, after the department determined that the incident could have been averted. "The bottom line is this tragedy was preventable," said Judy Schurke, department director. Tesoro said it would not comment until the company studies the findings.
