Repair and maintenance professionals shouldn't stop at diagnosing the reason that equipment failed, writes pharmaceutical blogger "The QA Pharm." Mistakes made by individual people are the root cause of many accidents and breakdowns, he writes, and repair teams are doing their company a disservice if they fail to name names. "Some say that the problem is always with the system, never a person," the blogger writes. "I doubt that, unless we run our businesses with robots. Even then, someone had to program them."
