Lawmakers consider means-testing for some Medicare costs

Breaking with history on the fundamental operation of the Medicare program, lawmakers are said to be close to agreeing on a provision under which beneficiaries with more income should pay more for doctor visits and nonhospital costs. Separately, nearly all 41.7 million Medicare patients will see their monthly premiums rise 13.5% next year.
