NPs can develop coordinated care plans for frail elderly

Nurse practitioners can improve the health of frail seniors by developing individualized care plans that coordinate services, according to Renee Roberts, an NP at Evercare Georgia and a leader in the field. Plans should include evidence-based interventions that prevent, diagnose and manage frailty; treatments for underlying disease; and diagnosis and treatment of depression.
