Exchange-sold health plans may vary after essential benefits are covered

All health insurance plans sold via Affordable Care Act marketplaces must cover a set of essential health benefits, but plan models and other benefits may vary. High-deductible plans may be linked to a health savings account if the individual deductible is at least $1,250 and the maximum out-of-pocket limit is $6,350. Some plans may offer coverage for fertility treatments and other services not deemed essential under the law.
