Self-care engagement may lower odds of readmission

Patients who had the lowest Patient Activation Measure scores, which evaluate engagement levels in self-care, had nearly twice the readmission rate of those who had the highest scores. A researcher said that while the measure is a vital indicator of rehospitalization, it should be used with other markers to determine post-discharge care plans. The findings appear in the Journal of General Internal Medicine.
