Medicare inspectors fail to report hospital errors

Hundreds of hospital medical errors found by Medicare inspectors are not reported to rating agencies, according to a report by HHS Inspector General Daniel Levinson. He also said no one follows policy changes to see if they are effective or determines if hospitals correct mistakes. He recommended CMS evaluate compliance using quality-assurance measures and that states monitor hospital improvement plans.
