Family physicians may benefit from population health approach, report says

A report from the Patient-Centered Primary Care Collaborative said the patient-centered medical home model is at the center of a population health approach that will use health information technology to drive collaborative efforts among a medical neighborhood made up of doctors, hospitals, home health, mental health and community organizations. Co-author Michelle Shaljian said if family physicians adopt a population health philosophy it could create a bridge "between the patients they've been taking care of for so long and the rest of the community."
