Employers struggle to provide insurance; deductibles rise

The number of Americans choosing health plans with deductibles of at least $1,000 has almost tripled over the past four years to about 20 million, a survey indicates. Physicians say they worry that the extra out-of-pocket costs that come with the less-expensive coverage may cause people to delay or skip routine care and needed screenings. Companies say they have little choice about raising premiums because of their own rising costs for materials and labor.
