Memorial Hermann Health System in Houston found uninsured patients resulted in hundreds of millions of dollars in uncompensated care and had high readmission rates, so officials created a master plan to provide better care for at-risk patients. Hospitals use patient risk stratification software combined with case management for post-discharge follow-up and also focus on medication compliance and patients who need durable medical equipment.
