Researchers propose quality measures for complex patients

Researchers have developed a five-measure framework designed to boost health care quality during transition in patients with complex conditions. The framework recommends tracking measures such as 30-day readmissions rates, time between discharge and home care, and drug reconciliation at admission and discharge, researchers said. The findings appear in BMJ Quality & Safety.
