Mass. health system attempts to cut readmissions with analytics tool

UMass Memorial Health Care is using transaction analytics software to track patients admitted to its five hospitals in an effort to reduce readmissions for patients with heart attacks, heart failure and pneumonia. The system was penalized by Medicare for high readmission rates and the software is designed to flag at-risk patients, prompting physicians and nurses to do follow-up care.
