CMS says pilot program reduces hospital readmissions

CMS officials said a year into its Care Transitions project to reduce rehospitalizations among Medicare patients, facilities in 14 states are beginning to see those numbers decline. The programs focus on ensuring patients take their medications and get needed follow-up care so problems are caught early and don't lead to hospitalization.
