Survey sheds light on limited use of hospital reporting systems

A recent survey conducted by the Agency for Healthcare Research and Quality found that 96% of hospitals implement adverse-event reporting systems, but only 21% use them to fully distribute error reports throughout their organizations. The agency is planning to use the survey results to develop baseline data on hospital reporting systems as it assesses their effectiveness for improving health care.
