One-third of adult patients don't get post-discharge care, study says

Researchers from the Center for Studying Health System Change found that around one-third of patients discharged from hospitals do not consult a nurse practitioner, physician or physician's assistant within 30 days. The results suggest that there is a significant coordination gap with community health providers, and this could affect efforts to cut hospital readmission rates.
