Industry is not learning from past disasters, CSB finds

The oil and natural gas industry is not taking the necessary measures to avoid disasters, the Chemical Safety and Hazard Investigation Board said in its probe of the Gulf of Mexico rig blast and oil spill. "Despite significant progress, not all the lessons of [the fatal refinery accident at] Texas City and other CSB investigations have been effectively implemented by the oil industry," said Don Holmstrom, the board's lead investigator.
