Care Management Plus improves outcomes for seniors

A three-year study that included more than 3,400 chronically ill seniors shows using the Care Management Plus model reduced death rates and hospitalizations. Oregon Health & Science University researchers say the program uses nurse care managers, interdisciplinary teams, and electronic tracking and care coordination systems. In the first year of the study, 6.5% of CMP patients died compared with 9.2% of those in a control group.
