Experts propose solutions to improve patient safety

Patients do not receive recommended care in about 50% of cases due in part to some health care providers' reluctance to adopt standards that may not be well established, according to patient-safety experts. They suggest that providers be more involved in team-oriented medicine and in establishing standards and that medical students and residents be trained in team decision making.
