A study in the Annals of Family Medicine showed that 211 of 1,098 patients who had one or more needs related to social determinants of health documented in the EHR had a related referral in their EHR, and only 15% to 21% of those with a document need reported wanting help to address the need. Researchers recommend clinicians evaluate whether EHR upgrades or other changes affect tool use and/or require more training, develop workflows that will enable data to be entered directly in the computer until the data are included in the EHR and ensure that individual patient data are reviewed in workflows.
