Medicare's program to penalize hospitals for readmitting too many heart attack, heart failure and pneumonia patients within 30 days took effect on Monday. The penalties, part of the Obama administration's push to improve quality of health care, are expected to hit about 2,200 Medicare-affiliated hospitals.
