The Camden Coalition of Healthcare Providers in New Jersey is working to reduce costs and patient loads by making sure so-called ER "super users" get basic health care outside of a hospital setting. The coalition hired a nurse practitioner, social worker and community health aide for an experimental program to offer in-home health care and social services to 60 patients identified as the biggest nonemergency users of ER services.
