A fatal helicopter accident in Arizona was caused by a faulty rotor blade that came apart in the air, according a lawsuit filed by the pilot's family. Witnesses saw pieces coming off the Eurocopter EC135 just before its emergency landing north of Phoenix. The pilot, a U.S. Army veteran, had flown more than 11,000 flight hours over the past 30 years.
