The American Heart Association said the transition of adolescents with congenital heart defects from pediatric to adult care should actively involve the patients and consider their emotional and developmental preparedness. Youths with CHDs should learn independent care of themselves during childhood, be provided with a transition curriculum about the heart disease and be moved to adult CHD care only after they are ready to meet their needs independently, the AHA said.
