The Voices of Detroit Initiative used outreach workers and patient navigators to find medical homes for 55% of 33,000 uninsured patients who otherwise would have opted for more costly care at hospital emergency departments, says Dr. Herbert Smitherman, Wayne State University's assistant dean of community and urban health. Detroit has lost 60% of its primary care physician capacity since 1997 and Smitherman says the city must rebuild its provider capacity to deal with increased numbers of patients coming in through health law changes and the expansion of Medicaid.
