The rate of rehospitalization after discharge from the Hebrew Rehabilitation Center in Boston dropped from 16.5% to 13.3% after the center's skilled nursing facility adopted intervention policies that included guidelines for common geriatric syndromes and consultations with patients who had been frequently hospitalized recently. The percentage of patients discharged to their homes rose from 68.6% to 73% while the rate of those sent to long-term care after hospitalization decreased from 13.8% to 11.5%, according to a study in the Journal of the American Geriatrics Society.
