Hospital patients, especially those in the ICU, often do not get the medications they regularly take at home for chronic conditions and many fail to restart them after discharge, researchers reported in the Journal of the American Medical Association. University of Toronto researchers looked at data on about 400,000 elderly patients taking stomach acid, cholesterol, blood thinning, thyroid and respiratory inhaler drugs and found discontinuation was mostly unintentional and that better communication among care teams and patient follow-up are needed.
