Electrocautery during Mohs surgery can cause inflammation evidenced by an abundance of neutrophils. However, chronic inflammation within or near a neurovascular bundle could be a sign of a perineural tumor that can be difficult to spot, says dermatologic surgeon Alexander Miller. "When there's inflammation, one needs to look very carefully at all sections of the slide," Miller said.
