A study in JAMA Internal Medicine calls for increasing EHR reviews to help providers prevent breakdowns during consultations that lead to diagnostic errors. The study, which reviewed cases from a large Department of Veterans Affairs facility and integrated private health care system, found that the diseases frequently misdiagnosed included urinary tract infection/pyelonephritis and pneumonia. "Most errors were associated with potential for moderate to severe harm," according to the authors.
