Physician billing for Medicare should include new evaluation and management codes that recognize the complexity and intensity of primary care office visits, according to a letter to the Senate Finance Committee from AAFP Board Chair Glen Stream, M.D. He also called on Congress to encourage the adoption of the patient-centered medical home model by paying PCMH practices monthly per-patient fees for management and coordination of patient care inside and outside of the office setting.
