Home monitoring devices given to heart failure patients send daily health data to nurses, who may spot problems early and help prevent hospital readmissions. Nurse practitioner Linda Wick, program manager of the heart failure program at Essentia Health in Duluth, Minn., said the monitors are tools and the more important part is that nurses are talking to patients to assess their medical conditions and determine whether they are complying with treatment plans.
