Conversations about end-of-life care between physicians and patients are not highly valued by the U.S. health care system, but bills now in Congress that would have Medicare reimburse for such services could start to change that, writes oncologist Mary Mulcahy. When cancer therapy is no longer working, care goals can change, and she is proud when terminal patients "take control of their lives" and set out advance directives.
