Patients who had the lowest Patient Activation Measure scores, which reflect engagement in self-care, were nearly twice as likely to be readmitted as those who had the highest scores, a study found. Fairview Health Services in Minnesota has been using the scores since 2009, but a hospital official noted the metric should be used with other markers to develop postdischarge care plans. The findings appear in the Journal of General Internal Medicine.
