A Yale University study found that from 2010 to 2012, many hospitals changed their policies to prevent readmissions, with more of them partnering with other local hospitals, discharging patients with a follow-up appointment already in place and tracking patients after discharge. Study author Leora Horwitz said while many hospitals made significant efforts to reduce readmissions, "the absolute number of hospitals in the country working on these sorts of efforts in most cases is not even half."
