Dermatologic surgeon Patricia Wexler says she injects most fillers above the periosteum and focuses less on volumizing than on shaping as more patients seek an oval or heart-shaped face instead of a round face. She gives most fillers around the periphery because little volume or elasticity is lost in the center of the face. Combining fillers with radiofrequency or ultrasound can reduce the signs of aging by 10 years or more, Wexler said.
