The University of Pennsylvania is using a three-year, $4.3 million grant from the Center for Medicare and Medicaid Innovation to test a palliative care program designed to give patients quality care outside the hospital while reducing health care costs. The Comprehensive Longitudinal Advanced Illness Management, or CLAIM, program provides patients with 24-hour access to medical personnel and home visits to manage care and acute episodes needing in-person attention.
