Increases in health insurance premiums over the last year were due primarily to the rising cost of care, a Commonwealth Fund analysis found. Higher prices for drugs, exams and procedures, along with providers' increased market power, led to premium hikes, the study found. Insurers have responded by implementing policies that encourage higher utilization of generic drugs and shifting to managed care and preferred network models in an effort to manage costs.
